This phraseology includes the installation of ceramic and mosaic tiles outdoors for artistic, decorative, and ornamental purposes. Fireproof tile construction Separately classify fireproof tile construction. Differentiation of Code 5604 from Code 5606 Code 5604 is differentiated from Code 5606 because Code 5606 is assigned to employees who have only administrative or managerial responsibility for construction projects by exercising operational control indirectly through full-time job supervisors or foremen of the employer. These employees include • construction executives • construction managers • construction superintendents, and • project managers. Division of payroll Code 5604 is not available for division of a single employee's payroll with any other classification. Important If the individual's duties originally assigned to Code 5604 change during the term to include actual construction, this code is not available for use. Laborers who perform punch-out list work Refer to the appropriate construction code for laborers who perform work to complete tasks that have been identified as part of a post-construction punch-out list. Door and window installation in connection with the construction of residential dwellings or commercial buildings The installation of doors and windows in connection with the construction of residential dwellings or commercial buildings is to be separately rated. Well Drilling--Water & Drivers BM-5402-HON0D_TN This phraseology includes the following operations up to completion of the substructure only: (a) concrete work, (b) excavation, (c) masonry, and (d) pile driving. BM-6217-RIFDE_TN This phraseology includes terrazzo work. Assignment of Code 5215 and Code 5222 with Code 5221 Code 5215 and Code 5222 must not be assigned to the same job or location where Code 5221 applies. Construction operations to be separately rated Construction operations to be separately rated include • airport runways • self-bearing floors • streets or roads, and • warming aprons. Construction--Elevator or Hod Hoist Installation, Repair, or Removal & Drivers--Iron or Steel Buildings or Structures This phraseology applies to specialist contractors who install all types of siding. Installation of siding on residential dwellings not exceeding three stories in height Refer to Code 5645 for the installation of siding on residential dwellings not exceeding three stories in height. Installation of sheet metal siding on the exterior of a steel frame structure Assign to Code 5535 the installation of sheet metal siding on the exterior of a steel-frame structure. Eligibility conditions for assignment to Code 5606 When determining eligibility, it is the job duties and not the job titles that are the main consideration. The following conditions must be met for assignment to Code 5606: 1. Project manager, construction executive, construction manager, or construction superintendent are defined as those persons exercising operational control indirectly through full-time job supervisors or forepersons of the employer. It is the job duties, and not the job titles, that are the main consideration. 2. When exercising control through a subcontractor, each subcontractor must have a job supervisor or foreperson at the specific job site in order to permit the assignment of this classification. The foreperson or supervisor of the subcontractor may manage one site or multiple sites. If any of the subcontractors do not have a job supervisor or foreperson at any job site visited by the construction executive, all of the payroll of the construction executive for that policy year is assigned to the highest rated construction class code applicable. A sole proprietor or owner/operator with no employees, working as a subcontractor for the employer, would prevent the assignment of this classification to a construction executive because the subcontractor does not have the required job supervisor or foreperson. 3. This phraseology does not apply to any person who is directly in charge of actual construction work or who is performing any degree of actual construction work. Such person must be assigned to the classification that specifically describes the type of construction or erection operation they are exercising direct supervisory control over, provided that separate payroll records are maintained for each operation. Any such operation without separate payroll records must be assigned to the highest rated classification that applies to the job or location where the operation is performed. 4. Code 5606 is not available for division of a single employee's payroll with any other classification. Vault Construction or Installation. Oil or Gas--Well--Perforating of Casing--All Employees & Drivers This phraseology includes all carpentry work in connection with the construction of residential dwellings when performed by employees of the same carpentry contractor or general contractor responsible for the entire construction project. Carpentry work Carpentry work includes • all interior wood trim • all types of roofing materials • cabinet installation • construction of the sill • decking • detached structures • doors • fencing • finished flooring • lathing • rafters • roof deck • rough floor • rough framework • sidewall sheathing • siding • stairs • wallboard installation • windows • wood or light-gauge steel joists, and • wood or light-gauge steel studs. Residential dwellings Residential dwellings include attached multiunit, detached, and semidetached residential buildings. Construction operations separately rated to Code 5403 The construction of the following buildings must be separately rated to Code 5403: • commercial buildings or structures, including mixed-use buildings, with no height restriction, and • multiunit residential buildings exceeding three stories in height. Iron or Steel--Erection--Construction of Dwellings Not Over Two Stories in Height BM-5057-CON64_TN This phraseology applies to cable installation in conduits or subways. Conduit construction Conduit construction is to be separately rated to Code 6325. Concrete operations The following operations must be separately rated to the appropriate concrete construction code: • the manufacturing of concrete piles at the job location, and • the pouring of concrete into hollow steel piles. Caisson work Pile-driving operations performed in connection with caisson work are to be separately rated to Code 6252. Marine and nonmarine construction The use of Code 6003 is allowed when pile driving is performed on any marine or nonmarine construction. Coverage under the United States Longshore and Harborworkers Act Use the state-approved factor found in the loss costs or rates for construction work where an employer obtains coverage under the United States Longshore and Harborworkers Act. This phraseology applies to domestic refrigeration pipe fitting including installation of tubing. Domestic refrigeration systems A domestic refrigeration system is a type of refrigeration system that • has a capacity that is 20 cubic feet or less • has a motor that is 1/3 horsepower or less • may be in a commercial or residential setting, and • is reach-in. BM-6206-OFF91_TN Concrete Work--Incidental to the Construction of Private Residence This phraseology applies only to a permanent yard maintained by a construction or erection employer for the storage of material or the storage and maintenance of equipment. Code 8227 includes incidental pickup or delivery of parts. This phraseology is not available at a construction site. Code 8227 and division of payroll Code 8227 is not available for division of a single employee's payroll during a single day. Operations not assigned to Code 8227 The following operations performed by construction site workers are incidental to the overall construction operation and must not be assigned to Code 8227: • loading and unloading equipment, materials, and tools • maintenance on equipment or vehicles, and • prefabrication work. Fabrication or mill operations Fabrication or mill operations are to be separately rated. Code 3724 must not be assigned to employees who perform the installation or repair of electrical apparatus on their employer's premises. Such payroll must be included in the governing classification. Operations to be separately rated The following operations are to be separately rated to Code 7538: • erection of poles • installation of service transformers on poles or on the outside of buildings • making of service connections, or • stringing of wires.
